Block by Block is a 10-week program where volunteers work one-on-one with families to identify their needs and connect them to resources and items of assistance. This program allows us to identify neighborhood needs, allowing for resident needs to be responded in both an individual and communal way.

Since July 2017, the Block by Block program has been implemented in six blocks with 45 families.

HOW BLOCK-BY-BLOCK WORKS
The timeline below is an overview of the program that explains what participants should expect.

Week 1 INTRODUCTIONConnector visits participant’s home and signs them up to participate in the Block-By-Block program. They complete the Block-By-Block Survey and the Household Needs Assessment, which give Connectors a better idea of the resources and items that can assist the participant.
Week 2
RESOURCE PACKAGE
During this week, the Connector puts together a Custom Resource Package, which consists of resources and items specific to the needs the participant identified in Week 1.
Week 3-4
DELIVERY
Participants can expect a phone call from their Connector during this time. The purpose of the call is to schedule a date and time to deliver the Custom Resource Package. The Connector meets with the participant to go through everything in the package and answer any questions they may have.
Week 5-6
FOLLOW-UP
Participants can expect a phone call from their Connector during this time. The Connector will follow up with the participant to ask about their experiences with the resources they were given. This is their chance to inform the Connector of their progress, provide feedback, and ask any additional questions they may have about the Resource Package. If there are additional resources the participant is interested in, they will let the Connector know at this time.
Week 7-9
CHECK-IN
Participants can expect a phone call from their Connector during this time. The Connector will follow-up with the participant via phone, as needed, to check in on their progress and gather any feedback on their experience thus far.
Week 10
EXIT SURVEY
Participants can expect a phone call from their Connector during this time. The Connector will schedule a date and time for one last meeting with the participant. When they meet, the participant will complete the Block-By-Block Exit Survey and provide feedback on their experience participating in the Block-By-Block program.
